Job description

JOB SUMMARY: Responsible for delivering case management/counseling services for clients and families. Monitors and provides oversight to other case managers in the Rapid Re-housing program.

ESSENTIAL FUNCTIONS:

  1. Designs, implements and monitors individual program plans for clients.
  2. Assesses client needs; investigate their eligibility for services.
  3. Counsels and aids individuals and families requiring agency assistance.
  4. Maintains client records and completes required documentation according to agency and accreditation standards. This may include, but is not limited to data entry, progress notes, activity reports, time sheets and logs.
  5. Coordinates client activities.
  6. Assists clients with accessing other services (job training, counseling, housing, medical services, etc.)
  7. Networks with other providers to generate community resources for participants.
  8. May provide supportive counseling in individual and group settings.
  9. May transport consumers to appointments, etc.
  10. Trains other case managers with regard to best practices for work products such as case tracking and data entry in Homeless Management Information System.
  11. Provides counsel to other case managers in duties associated to the provision of case management services.
  12. Acts as program liaison between case managers and Director.
