Case Manager - His Caring Place - 4KIDS
Are you passionate about walking alongside women in difficult seasons of life? Do you feel called to help mothers facing unplanned pregnancies or homelessness build stability, independence, and hope? 4KIDS is seeking a compassionate Case Manager who will provide guidance, accountability, and encouragement as mothers work toward personal goals while building a healthy future for themselves and their children.
Key Responsibilities:
  • Develop and oversee individualized case plans for residents
  • Conduct interviews and tours for prospective residents and participate in intake decisions
  • Maintain accurate resident documentation including individualized service plans (ISP), progress notes, and compliance paperwork
  • Orient new resident to the His Caring Place program
  • Facilitate case reviews and resident staffings with program staff
  • Assist residents with employment readiness, job placement, and career planning
  • Connect residents with community resources, education programs, and supportive services
  • Lead life-skills and educational group sessions
  • Provide guidance and support as residents navigate personal challenges and build healthy relationships
  • Develop discharge plans and provide referrals to long-term community resources
  • Prepare required monthly and quarterly reports
  • Provide program phone coverage and coordinate referrals as needed
  • Participate in outreach events, fundraising activities, and occasional evenings events for residents
Qualifications:
  • A bachelorโ€™s degree in Social Work or related field
  • Minimum two years of experience in case management, counseling, or social services
  • Experience in a Christian ministry setting demonstrating compassion, Christ-centered service, and understanding of trauma-informed care
  • Strong verbal and written communication skills
  • Excellent administrative, organizational, and documentation skills
  • A heart for serving children and families
  • Ability to lift up to 25 pounds
  • Valid Florida driver's license with no more than three points
