Registered Nurse Case Manager (RNCM)
Pay:
 $89,000.00 - $91,000.00 per year

What You Will Do:

  • Deliver compassionate, skilled nursing care wherever our patients call home.
  • Collaborate with interdisciplinary teams to coordinate patient care and facilitate effective communication among providers and team.
  • Provide clinical, emotional and educational support to families and caregivers.
  • Advocate for patients by providing education on their conditions, treatment options, and available resources.
  • Work closely with a team of professionals committed to dignified care.
