We are seeking a compassionate and organized Registered Nurse Case Manager to support our non-medical home care clients. This role is focused on client assessments, care plan oversight, and caregiver supervision. This is not a skilled nursing or hands-on clinical care position.

The RN Case Manager will help ensure clients receive safe, appropriate, and personalized care from our team of professional caregivers.

Key Responsibilities
  • Complete initial and ongoing client assessments in the home.
  • Develop and update non-medical care plans based on client needs.
  • Oversee and support caregivers assigned to client cases.
  • Monitor client condition and communicate changes to office leadership and families as needed.
  • Ensure caregiver services align with the approved care plan.
  • Provide guidance and coaching to caregivers on client-specific needs and expectations.
  • Document assessments, care plan updates, and case notes accurately and timely.
  • Assist with quality assurance visits and service reviews.
  • Maintain compliance with company policies and applicable state home care requirements.

Qualifications
