Job Summary and Qualifications 

* This is a PRN Day Shift position.  Hours are typically 8:00am - 4:30pm *  

As a Case Manager, your role will be to support patients and families through every step of their care journey. You will coordinate services, connect resources, and develop care plans that reflect each patient’s unique needs. By partnering with physicians, nurses, and department leaders, you will help ensure safe transitions, clear communication, and consistent quality across the continuum of care. 

The Case Manager shall be responsible for monitoring the patient’s plan of care as developed by the physician to insure appropriate services are accomplished in a timely fashion.  The case management process is initiated prior to admission when possible, and extends across the continuum of care until the patient no longer requires management. The intensity of services provided is dependent upon the patient’s needs and the resources available to produce the desired outcome. The case manager works in concert with the physician and staff to ease the patient’s transition through the continuum and optimize resource utilization. The Case Manager coordinates the interdisciplinary team approach in assessing the need for discharge planning/case management. Also, is a resource to staff members on community services and resource management. Case Manager should be able to work independently with minimal supervision. Case Managers report to the Director/Manager of Resource Management.

What qualifications you will need:
  • Current RN license in the state of Louisiana or Registered Nurse diploma
  • 2-3 years nursing experience in an acute care setting, case management experience preferred
  • Certification in Case Management, Nursing, or Utilization Review preferred
