Job Title: Social Services Intern

Program: ICMSG/PATHG/Bridge

Reports to: Director of Case Management and Community Support Services

Part-Time 15-20 hours per week. Flexible hours, 3-5 days/week

Supervisory Responsibilities:

None

Program Overview:

The Integrated Case Management Service (ICMS) is an outreach program serving individuals 18 years and older who reside in Gloucester and Cape May Counties. Services will be available to all eligible consumers with the intent to eliminate barriers to service regardless of location (in catchment areas) and cultural or linguistic orientation. The ICMS program includes targeted case management services.
ICMS assist consumers to reach and maintain their optimal level of functioning in the community.

Position Summary:

The Social Services intern has the primaryresponsibilitysupportive service coordination, provision or arrangement of services needed, personal advocacy and development, training on service plans, and support to team with initial intake and referral outreach for client engagement. The intern provides face-to-face interaction with the clients supporting the linkage contact necessary to achieve the client's case management goals.

As an integrated healthorganizationAcendavalues the use of evidence-based practice or EBPs.

Duties and Responsibilities:

  • For all below duties, a bachelor's level intern will have a more supportive role to the current case manager.
  • The intern interacts with clients and supports the team in meeting service goals. Services include engagement strategies, activities to meet meaningful goals over time, and linkage to resources.
  • Demonstrates competent written and verbal communication with referral sources and external customers.
  • Attentive and responsive to customer needs in a helpful,supportiveandtimelymanner.
  • Works collaboratively with coworkers and agency staff to effectively communicate andproblem-solve.
  • Meets obligations of the program (contractual, licensing, Medicaid, etc.).
  • Follows program policies and procedures and encourages the same in others.
  • Educatesself onissuesimpactingthe program population.
  • Assists with operating conscientiouslyin accordance withprogram budget (i.e., use of client engaging funds, office supplies).

Education, Work Experience and/or Skills Requirements:

  • Currently enrolled in a Bachelor's or Master's level program at a NJ Institute of Higher Learning
  • Must have and maintain a valid driver's license with an acceptable driving record
  • Must be able to exercise independent thinking and good judgment under all circumstances.
  • Must be able to analyze work, set goals, develop plans and utilize time effectively and efficiently.

Physical Requirements:

  • While performing the duties of this job, the employee is occasionally required to remain in a stationary position, often standing or sitting for prolonged periods.
  • Repeated motions that may include the wrists, hands and/or fingers
  • The employee must occasionally move about to accomplish tasks, adjust or move objects up to 20 pounds.
