Location: 2 Wisconsin Circle Suite #230 Chevy Chase Maryland

Schedule: Monday - Friday, 9:00am - 5:00pm

Embark Behavioral Health is seeking a compassionate Case Manager to support our Intensive Outpatient Program (IOP) and Partial Hospitalization Program (PHP) in Rockville, Maryland.

As a Case Manager, you will lead the coordination of education success plans, conduct intake assessments, assist in facilitating PHP sessions, and manage ongoing communication with families, schools, and the clinical team. You will also support medication management, track attendance and treatment progress, and contribute to thoughtful discharge planning.

This is an ideal opportunity for school social workers, school counselors, student support specialists, educational case managers, and other education professionals who are passionate about helping adolescents and families and want to transition into a more clinical behavioral health setting.


  • Conduct education assessments upon intake and develop individualized Education Success Plans tailored to each student’s needs.

  • Coordinate and facilitate weekly or as-needed check-ins with students, while maintaining communication with school contacts to support academic accommodations and track attendance.

  • Attend staff meetings, and assisting with student conflict resolution and case management as needed.

  • Collaborate with the clinical team, parents, and the Enrollment Director to create treatment schedules, manage pre-treatment processes, and plan for discharge.

  • Oversee medication management coordination to ensure accurate and consistent delivery of prescribed medications.

  • Provide access to essential health services by assessing student needs and working with administrative staff to schedule medical, dental, and vision appointments.

  • Maintain timely and accurate documentation in Best Notes, conduct parent outreach for student updates, and support students in learning life skills and household responsibilities.


  • Education & Experience: Bachelor’s degree in Behavioral Sciences, Education, or a related field required; experience in education and behavioral health strongly preferred.

  • Working knowledge of DSM-V disorders, ability to develop and adjust treatment plans, and experience supporting students and families in crisis with professionalism and empathy.

  • Strong verbal and written communication skills, ability to set and maintain healthy boundaries, and a calm, supportive approach to conflict resolution.

  • Proven ability to manage schedules, prioritize tasks, meet deadlines, and adapt in a fast-paced, evolving clinical environment.

  • Must be CPR and First Aid certified and able to pass a state background check.
