Hospice RN Case Manager

Specialty/Department: Nursing – Hospice RN / Care Manager
Location: Barre, VT 05641
Job Type: Contract
Duration: 20 Weeks
Shift: Day Shift
Schedule: Monday – Friday
Guaranteed Hours: 36 Hours Weekly
Start Date: Typical start date is within approximately 2 weeks after offer acceptance and completion of onboarding/credentialing requirements.

Experience Requirements
  • Minimum 1 year of RN experience required
  • Hospice case management experience required
  • Home hospice care experience required
  • Rural travel experience preferred
  • Experience with McKesson/Netsmart EMR preferred
License & Certification Requirements
  • Active Vermont RN License required
  • BLS/BCLS certification required

Seeking an experienced Hospice RN Case Manager for a home health and hospice assignment in Vermont.
