What You'll Do
  • Track and record patient case information, progress, and evaluate recovery.
  • Meet with patient and their families to understand their needs and recommend appropriate care with treatment plans, while providing support and reassurance.
  • Work with team members to ensure discharge planning goals and objectives are developed.
  • Schedule appointments, and coordinate care
