Job Description
We are seeking a highly experienced Acute Inpatient Case Manager to join our team. This role requires a strong background in acute inpatient medical case management and discharge planning within a Level 1 Academic Medical Center. The candidate must be adaptable, able to pick up processes quickly, and work efficiently in a busy environment.
Responsibilities
  • Manage discharged patients, ensuring appropriate interventions and support tailored to a variety of medical diagnoses.
  • Collaborate with a team of social workers and other healthcare professionals to facilitate patient care.
  • Conduct assessments and plan discharges, coordinating with local facilities as needed.
  • Utilize EPIC charting for documentation and communication of patient care activities.
  • Responsible for a caseload of 15-20 patients per day, covering all inpatient units throughout the hospital.
Required Experience / Certifications / Licensure
  • Minimum of 2 years of Acute Inpatient Medical Case Management experience, with at least 1 year in a large, busy hospital emergency department (500+ beds, Level 1/Academic Medical Center).
  • Recent discharge planning experience with a range of medical diagnoses within the past two years.
  • Strong computer skills with proficiency in EPIC charting.
  • Current BLS certification.
