What are the key skills and qualifications needed to thrive as a case manager, and why are they important?

To thrive as a Case Manager, you need strong assessment, planning, and organizational skills, often supported by a degree in social work, nursing, or a related field. Familiarity with case management software, electronic health records, and relevant certifications such as CCM (Certified Case Manager) are typically required. Exceptional communication, empathy, and problem-solving abilities help you build trust and effectively advocate for clients. These skills ensure comprehensive, client-centered care and successful outcomes in complex, multidisciplinary environments.

How does a case manager typically collaborate with other professionals to support clients?

Case managers often work closely with a multidisciplinary team that may include social workers, healthcare providers, counselors, and community resource coordinators. They act as a central point of contact, facilitating communication between all parties to ensure clients receive comprehensive and coordinated care. Regular meetings, case conferences, and detailed documentation are common practices to track progress and address any challenges. This collaboration is essential for developing effective care plans and achieving the best outcomes for clients.

What qualifications do you need to be a case management?

To become a case manager, candidates typically need a bachelor's degree in social work, healthcare, or a related field. Relevant skills include strong communication, organization, and problem-solving abilities, and some roles may require certification such as the Certified Case Manager (CCM) credential. Experience in social services or healthcare settings is also often preferred.

What is case management?

Case managers work with clients to ensure they receive the appropriate resources, support, and care, often acting as a liaison between clients, families, and service providers. This role is common in healthcare, social services, and legal fields, aiming to improve outcomes and promote client well-being.

Job description

Our Why
Fourth Street Clinic helps Utahns experiencing homelessness improve their health and quality of life by providing high-quality, integrated healthcare. Our team is passionate, collaborative, and deeply committed to health equity and human dignity.
What You'll Lead
As Case Management Director, you'll set the clinical and operational direction for our Case Management program-making sure services are consistent, high-quality, and grounded in trauma-informed, harm-reduction, whole-person care. You'll coach supervisors, align practice standards, track quality/performance outcomes, and keep teams connected across clinic, outreach, and community settings-while staying compliant with organizational and regulatory requirements.
Required
  • Master's in Social Work, Mental Health Counseling, or related field
  • Active Utah licensure (LCSW, CSW, ACMHC, or CMHC)
  • 5+ years medical and/or behavioral health case management (or related)
  • 5+ years leadership experience; able to supervise supervisors/managers
  • Experience with high-acuity, medically complex populations and homelessness/behavioral health/SUD
  • EHR proficiency; strong communication, conflict resolution, and change leadership
  • Trauma-informed, harm-reduction, health equity commitment
  • Valid UT driver's license + insurability
Ready to lead with heart and systems-thinking? Apply today.

A Typical Weekday
You'll be onsite Monday-Friday, 8:00-5:00, starting with a quick pulse-check on program metrics and any overnight updates from outreach. Your day blends big-picture planning with real-time support: huddles with Case Management Supervisors, quick consults with multidisciplinary partners, and purposeful check-ins that keep communication smooth across clinic and community sites.
Between meetings, you'll review dashboards and documentation patterns, spot trends, and tee up practical improvements the team can actually use. Expect a steady rhythm of collaboration-problem-solving barriers to care, keeping workflows steady during change, and making sure the program stays mission-aligned and audit-ready. By day's end, you'll have moved both people and systems forward (and probably answered a few "quick questions" that weren't quick).
The Good Stuff
  • Competitive salary: Starting at $105,000 DOE
  • Medical, dental, and vision coverage
    • 100% employer-paid health insurance for the employee; 80% dependent premium paid
  • 401(k) (eligible after 90 days)
  • Health Savings Account (HSA) and Flexible Spending Account (FSA)
  • Generous Paid Time Off and Paid Holiday schedule
  • Options across comprehensive benefit packages options, including Disability, Life, ER Paid Life, Accident, Hospital, Critical Illness.

Must be able to pass a background check, including fingerprints.
