Job description

Case Management Director

Fourth Street Clinic helps Utahns experiencing homelessness improve their health and quality of life by providing high-quality, integrated healthcare. Our team is passionate, collaborative, and deeply committed to health equity and human dignity.

As Case Management Director, you'll set the clinical and operational direction for our Case Management programmaking sure services are consistent, high-quality, and grounded in trauma-informed, harm-reduction, whole-person care. You'll coach supervisors, align practice standards, track quality/performance outcomes, and keep teams connected across clinic, outreach, and community settingswhile staying compliant with organizational and regulatory requirements.

  • Master's in Social Work, Mental Health Counseling, or related field
  • Active Utah licensure (LCSW, CSW, ACMHC, or CMHC)
  • 5+ years medical and/or behavioral health case management (or related)
  • 5+ years leadership experience; able to supervise supervisors/managers
  • Experience with high-acuity, medically complex populations and homelessness/behavioral health/SUD
  • EHR proficiency; strong communication, conflict resolution, and change leadership
  • Trauma-informed, harm-reduction, health equity commitment
  • Valid UT driver's license + insurability

You'll be onsite MondayFriday, 8:005:00, starting with a quick pulse-check on program metrics and any overnight updates from outreach. Your day blends big-picture planning with real-time support: huddles with Case Management Supervisors, quick consults with multidisciplinary partners, and purposeful check-ins that keep communication smooth across clinic and community sites. Between meetings, you'll review dashboards and documentation patterns, spot trends, and tee up practical improvements the team can actually use. Expect a steady rhythm of collaborationproblem-solving barriers to care, keeping workflows steady during change, and making sure the program stays mission-aligned and audit-ready. By day's end, you'll have moved both people and systems forward (and probably answered a few "quick questions" that weren't quick).
