Non-Clinical Case Manager (RN) – Arizona Army National Guard

Location: Phoenix, Arizona

Solvet is seeking a qualified Non-Clinical Case Manager to support the Army National Guard in Phoenix, Arizona. This position plays a vital role in coordinating non-clinical case management services for Service Members while ensuring compliance with military and healthcare program requirements.

Position Summary

The Non-Clinical Case Manager will provide comprehensive case management support services, assisting Service Members with care coordination, resource navigation, documentation, and follow-up activities. This role focuses on non-clinical support and requires strong organizational, communication, and case coordination skills.

Essential Duties and Responsibilities

  • Coordinate non-clinical case management services for Arizona Army National Guard personnel
  • Conduct case intake, assessment, tracking, and follow-up activities
  • Assist Service Members in identifying and accessing available resources and support services
  • Maintain accurate, timely, and confidential case documentation
  • Collaborate with military leadership, healthcare providers, and community organizations as appropriate
  • Monitor case progress and ensure continuity of support services
  • Prepare reports and maintain records in accordance with applicable regulations and contract requirements
