Job description

About the Role:

The MAT Connections Specialist plays a critical role in bridging the gap between patients seeking Medication-Assisted Treatment (MAT) and Ascensa Health. This position is focused on facilitating access to treatment resources, ensuring patients receive timely and appropriate care to support their recovery journey. The specialist will work closely with healthcare professionals, community organizations, and patients to coordinate services and provide education about MAT options. 

Minimum Qualifications:

  • CARES/ CPS-AD certification required
  • High school diploma or equivalent; Associate’s or Bachelor’s degree in healthcare, social work, or related field preferred.
  • Experience working in case management or addiction services.
  • Strong understanding of Medication-Assisted Treatment principles and addiction recovery processes.
  • Excellent communication and interpersonal skills to effectively engage with diverse populations.
  • Proficiency with electronic health records (EHR) systems and basic data entry.

Preferred Qualifications:

  • Bachelor’s degree in public health, social work, psychology, or a related discipline.
  • Certification or training in addiction counseling. CARES/ CPS-AD required.
  • Experience working within community health organizations or substance use disorder treatment programs.

Responsibilities:

  • Serve as a primary point of contact for patients seeking information and access to Medication-Assisted Treatment programs.
  • Collaborate with healthcare providers, social workers, and community organizations to support patient engagement and retention in treatment.
  • Provide follow up and appointment reminders for MAT patients. 
  • Track and document patient interactions and outcomes to support program evaluation and continuous improvement.
  • Provide education and outreach to patients and families about the benefits and processes of MAT.
  • Assist in identifying and addressing barriers to treatment access, including insurance, transportation, and stigma-related challenges.

The MAT Connections Specialist utilizes strong communication skills daily to build trust and rapport with patients and healthcare providers, ensuring clear and compassionate information exchange. Organizational skills are essential for managing multiple referrals, appointments, and follow-ups efficiently, which helps maintain smooth patient care coordination. Knowledge of healthcare systems and MAT protocols allows the specialist to navigate complex treatment landscapes and advocate effectively for patient needs. Data management skills support accurate documentation and reporting, which are critical for monitoring program success and identifying areas for improvement. Additionally, cultural competency and empathy enable the specialist to address stigma and barriers sensitively, fostering an inclusive environment that encourages patient engagement and recovery.
