YHDP Program Mission
The Youth Homelessness Demonstration Program (YHDP) is a housing-focused initiative designed to move youth experiencing homelessness into permanent housing while providing supportive services that promote long-term stability and self-sufficiency. Through a coordinated and integrated team that includes case managers, a peer support specialist, and a therapist, the program supports youth ages 18-24 in maintaining stable housing and working toward goals related to mental and physical health, education and employment, life skills development, and community connection.
Position Summary
The Permanent Supportive Housing Case Manager provides consistent, individualized, and trauma-informed case management services to youth and young adult households who have transitioned out of homelessness and are living in housing throughout Salt Lake County. The case manager will support the clients in obtaining and maintaining stable housing and addressing current needs as defined by the client. The PSH case manager position is housed out of the VOA Youth Resource Center and includes mobile work meeting with clients out in the community/in their homes.
Essential Duties
  • Provide individualized case management services to program participants, supporting goals related to housing stability, health care, mental health, education, employment, government benefits, parenting supports, and legal needs.
  • Develop collaborative service plans with participants that prioritize housing stability and support broader goals related to self-sufficiency and well-being.
  • Conduct initial and ongoing assessments of participant needs using agency-approved tools and coordinate appropriate services and referrals.
  • Maintain regular contact with participants through home visits, community meetings, and office appointments to support housing stability and service engagement.
  • Support participants in maintaining housing by assisting with landlord communication, tenancy responsibilities, and conflict resolution when appropriate.
  • Maintain complete and accurate client files including service plans, case notes, referrals, and discharge documentation.
  • Enter and maintain timely and accurate documentation in HMIS (UHMIS) and internal tracking systems in accordance with HUD, Continuum of Care (CoC), and VOA program standards.
  • Assist with documenting program participant eligibility requirements and maintaining compliance with grant and program regulations.
  • Collaborate with program staff and the Program Manager to ensure effective coordination of services, program fidelity, and documentation accuracy.
  • Attend and participate in weekly staff meetings and multidisciplinary case consultations to review participant progress and coordinate support strategies.
  • Demonstrate professional, trauma-informed, and culturally responsive interactions with clients, families, and community partners.
  • Utilize agency vehicles when appropriate to transport participants to appointments, housing viewings, or other essential services.
  • Assist with housing move-ins and transitions, including transporting or lifting boxes, household items, or small furniture as needed.
  • Participate in the Youth Resource Center on-call rotation and perform other duties as assigned to support program operations.
  • Maintain consistent, punctual, and reliable attendance.
  • Perform other duties as assigned.

  • Must be able to qualify for and obtain Utah Department of Human Services Case Management Certification within 60 days of hire.
  • Demonstrates clear communication skills and the ability to work cooperatively with others.
  • Ability to maintain professional boundaries, uphold confidentiality requirements, follow agency policies and procedures, and interact professionally with a diverse workforce, clients, and members of the public.
  • Ability to apply trauma-informed practices and exercise sound judgment within the scope of the position while seeking supervision when appropriate.
  • Willingness to accept supervision, feedback, and direction.
  • Ability to engage with clients for extended periods and move throughout the facility as required to provide services.
  • Must be at least 21 years of age, possess a valid driver's license, maintain a good driving record, and be insurable under the agency's liability policy.
  • Must pass Utah Department of Human Services, Office of Licensing background screening requirements for individuals working with vulnerable populations.
  • Proficiency in Microsoft Office and basic computer systems.

Physical Demands and Work Location
  • Ability to lift and carry up to 50 pounds to assist with client move-ins and other program needs.
  • Ability to move throughout the facility and engage with clients for extended periods.
  • Work may involve prolonged computer use.
  • This position must be performed on-site due to the need to provide direct client services, including meeting with clients in person, conducting home visits, transporting clients to appointments, and supporting access to services.
