Position Summary:

The Case Planner works collaboratively with therapists, education specialists, and social workers to manage a caseload of approximately 10 children with varying mental health needs. The role focuses on providing intensive casework and field services to support permanency goals, while also conducting outreach, coordinating Family Team Conferences, maintaining case records, utilizing the CONNECTIONS system, and participating in case consultations.

Essential Duties:

  • Understand and promote the organization's Mission, Vision, and Core Values to ensure alignment with organizational policies and procedures.
  • Work with a dynamic team of professionals including therapists, education specialists, and social workers in our foster care program for children.
  • Manage a case load of approximately 10 children with varied levels of mental health needs.
  • Provide intensive casework services and fieldwork services for children and their birth and foster families with the goal of providing permanency as quickly as possible.
  • Conduct outreach to connect children and families with agency and community treatment resources.
  • Utilization of CONNECTIONS, child welfare computer system.
  • Coordinate Family Team Conferences.
  • Actively participate in case consultations.
  • Maintain written case records.
  • Perform other duties as assigned.

Minimum Qualifications:

  • Bachelor's Degree required.
  • Foster Care experience preferred.
  • Bilingual (Spanish/English) preferred.
  • Valid Driver's License.
