Job Summary
The Case Manager provides case management services in either the community and/or in office setting to patients and their families. The Case Manager assists the patient and care team in planning, connecting, outreaching, advocating, coordinating and monitoring the patient's needs and progress. The Case Manager assists patients in gaining access to health services, financial assistance, employment, education, social services and natural support. The Case Manager assesses patient needs, develops and monitors individualized treatment plans and services provided in an accurate and timely manner. Embraces the recovery philosophy and promotes a safe recovery environment for clients.
Skills/Requirements
  • Highschool diploma or GED required. Associates Degree or higher in a field related to behavioral health preferred.
  • 1-3 years of experience working with the GMH/SU population required.
  • 1-3 years of experience in behavioral health position providing outreach and engagement activities required.
  • 1-3 years of experience in medical setting preferred.
  • Minimum of six months of recovery from substance use and/or mental health disorders preferred.
  • Current AZ Driver's License (valid and in good standing), preferred.
  • Valid ID required
  • Clean 39-month driving record (MVR) preferred.
  • Behavioral Health Technician (BHT) in accordance with A.A.C. R9-10-101.33 and CBI Clinical Policies and Procedures
  • Arizona Fingerprint Clearance card
