We are looking for dedicated CHHAs who want to provide quality care to home based patients. You will work one-on-one with the patient and truly impact their life as well as their family's. The job is consistent, long-term, and can work with a variety of different schedules.
The CHHA is responsible for providing direct patient care under the supervision of a registered nurse. Responsibilities include following the plan of care and working collaboratively with the members of the team to help meet positive patient care outcomes.

Certified Home Health Aide (CHHA) RESPONSIBILITIES:
  • Work closely with the RN case manager and follow instructions in providing care.
  • Perform all task delegated by the RN on the Care Plan
  • Immediately notify the RN case manager if any task is not performed or is outside of the Care Plan.
  • Perform task delegated by the RN such as but not limited to feeding, ADLS, socializing, ambulation, repositioning, etc.
  • Aid with hygiene (bathing, oral care, and dressing).

HOME HEALTH AIDE QUALIFICATIONS:
  • Current Home Health Aide Certification
  • CPR certification
  • Driver's license or valid ID
  • Proof of social security or legal work permit
