Job description

The RN Case Manager, MAT assesses and monitors the client's physical and psychological withdrawal from mood-altering chemicals and administers medications as prescribed with the goal of ensuring the client detoxifies safely.

Case management is a collaborative process which assesses, plans, implements, coordinates, monitors, and evaluates the options and services needed to meet an individual’s health needs, using communication and available resources to promote quality and cost effective outcomes.

The RN Case Manager, MAT will provide quality patient care via the development, implementation & evaluation of individual patient care plans. (S)he is a liaison between the patient, the family of the patient, community partners, and organization-wide within WSMC.  Follows established professional standards of care, WSMC guideline policy and procedures

MINIMUM QUALIFICATIONS

  1. Registered Nurse currently licensed in the State of California. BSN and/or CARN desirable.
  2. Experience in Substance Use Disorder preferred.
  3. Current BLS for Healthcare Providers with AED certification..
  4. Knowledge of basic nursing skills, including, but not limited to: patient assement,  education, examination procedures, use and maintenance of equipment, administration of medications, minor lab procedures including urinalysis, sterilization techniques of medical equipment, ability to use good judgment in situations dealing with confidential information and difficult patients, ability to triage patients, management of complex patient info and coordinating care.

DESIRED EXPERIENCE

  1. Familiarity with an electronic practice management system.
  2. Computer literate (i.e. logging on to computer, usage of keyboard and mouse, familiarity with Windows, Microsoft Word and Microsoft Excel).
  3. Ability to type 40 wpm
