Job description

CARING HEART NURSING AND REHAB has an immediate opening to join our growing team. SNF Case manager position available for experienced candidate to work on-site at our location. Candidate should have experience with managed care updates, authorization exclusions, Level 1 & 2,. Responsibilities include but are not limited to providing weekly updates to the insurance companies, assist with obtaining exclusions and carve outs, experience with the triple check process, medical terminology, medical classification and assisting the team with quality case outcomes. Must possess excellent customer service skills, good communication, and the ability to multi-task. Proficiency with Word & Excel is a Plus. The ability to be self-guided and directed would be beneficial. One to Two years experience with skilled nursing and Bachelor's Degree preferred.
