Position: Home Care Coordinator
Type: Full-Time
Schedule: Monday - Thursday 8:00 AM - 4:00 PM Friday, 8:00 AM - 1:00 PM
Location: Woodcliff lakes, NJ

We are seeking a compassionate and detail-oriented Home Care Coordinator to join our team and help us deliver high-quality care.

Key Responsibilities:

  • Oversee and coordinate home care services for clients, ensuring their needs are met with the highest standard of care.

  • Develop and maintain care plans in collaboration with clients, families, and healthcare providers.

  • Schedule and manage caregiver assignments, ensuring appropriate match between caregivers and clients.

  • Monitor client progress and adjust care plans as needed.

  • Address client and caregiver concerns and resolve issues in a timely and professional manner.

  • Maintain accurate and up-to-date records of care services provided.

  • Ensure compliance with all relevant regulations and company policies.

  • Work efficiently in a fast-paced environment, managing multiple tasks and priorities effectively.

Qualifications:

  • High school diploma or equivalent required; Associate’s or Bachelor’s degree in healthcare, social work, or a related field a plus.

  • Previous experience in home care coordination or a related field is highly desirable.

  • Strong organizational skills with a keen attention to detail.

  • Ability to work quickly and effectively under pressure.

  • Excellent communication and interpersonal skills.

  • Ability to work independently and as part of a team.

  • Proficiency in Microsoft Office and other relevant software.

  • Must be able to work Mon-Fri 8am-4pm in Suffern office.
