ResponsibilitiesAs a member of the multidisciplinary team, the Care Management Assistant provides discharge planning support to the Care Manager and Social Worker and supports patients and families / caregivers by reducing non-clinical barriers to discharge in the Inpatient and ED environments.QualificationsEducation
