Your Everyday

Collaborate with multidisciplinary team members to ensureappropriate directpatient care and implementation and documentation of care plan progress.

Maintain andadvocatea safe environment for everyone, report toappropriate careteam members, andinitiatecorrective actions.

Monitor and engage with your patients, families, andappropriate careteam members concerningsignificant events, patient condition changes, or patient questions and concerns.

Conduct rounds, answer calllightsand ensure adequateassistancewith meals, personal and oral hygiene needs.

Participate in the implementation of the plan of care.

Identify problems, concerns, changes in condition and report them immediately to appropriate care team personnel.

Work with nursing staff, physicians, families, consultantpersonneland ancillary service providers as directed by nursingsupervisor.

Ensure continuity of care, consistent communication, and an exceptional experience for every patient.

Follow established performance standards and duties according to policies and procedures and advise and consult with supervisor when unable to complete assignments or perform tasks promptly, effectively, or safely.

Initiate patient and family education usingappropriate resourcesand continuum of care services.

Contribute to the team byassistingothers without being asked.

The Must-Haves
Minimum:

High School Diploma,GEDor equivalent.

Louisiana Department of Public Health Certified Nurse Assistant license.

American Heart Association BLS certification.
