This provider will join one of the nation's most integrated healthcare systems, with over 1,400 providers in Indianapolis, IN. This opportunity is located on the north side of Indianapolis at Community Hospital North and Community Heart & Vascular Hospital.
Highlights
Community Physician Network is seeking a Nurse Practitioner or Physician Assistant, to work alongside our Pulmonary Hypertension team at Community Hospital North in Indianapolis, IN.
  • 4-10 hour shifts weekly, including in office and virtual home visits. Hours may vary based on patient volume/needs
  • Opportunity to practice in the Pulmonary Hypertension clinic, and inpatient hospital rounding.
  • Competitive compensation and benefits package
Qualifications
  • Experience practicing as a licensed Nurse Practitioner or Physician Assistant is required.
  • Pulmonary Hypertension, Heart Failure, or Palliative Care Experience is strongly preferred.
  • Must have excellent communication, critical thinking, and planning skills
  • Must be hard working, proactive, and dedicated to delivering high-quality patient care
  • Must be comfortable assessing patients independently
Licensure & Certifications
Educational background consistent with requirements for licensure and certification within the state of Indiana
