As a Licensed Clinical Social Worker, your day-to-day responsibilities will include:

• Developing personalized treatment plans that cater to the unique needs of hospice patients and their families, utilizing thorough clinical assessments.
• Actively engaging with a multidisciplinary team, including physicians, nurses, and other healthcare professionals, to forge a comprehensive treatment strategy that improves the patient’s quality of life.
• Monitoring patients’ emotional and psychological conditions, providing necessary follow-up to assess improvements and adapt treatment plans as needed.
• Offering counseling sessions to patients and their families, facilitating open discussions that address their concerns and needs during hospice care.
• Collaborating with educational and community resources to support patients, directly interfacing with school staff and caregivers to create supportive networks for younger clients.

This role specifically emphasizes care for hospice clients, many of whom are experiencing chronic illnesses or end-of-life transitions. You will be positioned to provide critical emotional support, cope with grief, and facilitate adjustments for families navigating complex feelings during this sensitive period. To succeed in this role, candidates must meet the following requirements:

• Active LCSW licensure is mandatory.
• Completion of a Bachelor’s Degree in Social Work or a related field.

• Strong verbal and written communication skills are essential for effective counseling and collaboration with clients and team members.
