Key Responsibilities
  • Conduct comprehensive evaluations and assessments to identify the mental health needs of clients, ensuring personalized care plans are developed.

  • Create and monitor individualized treatment plans that include scheduled therapy sessions, follow-up assessments, and progress tracking.

  • Engage actively in Interdisciplinary Team Meetings to discuss patient outcomes and strategize treatment adjustments.

  • Provide ongoing support through follow-up consultations, ensuring clients receive continual assistance tailored to their evolving needs.
  • Maintain accurate and organized records to ensure information is readily accessible to authorized individuals.
  • Act as a liaison between the community and the mental health services sector, raising awareness and promoting accessible care options.
Patient Population / Client Focus

Your role as an LPC will involve working with a varied demographic that includes children, adolescents, and adults facing a range of mental health challenges. Qualifications
