About the Role

Schedules and coordinates activities for individuals, both in their home and in the community. Additionally, plans specific activities to support individuals in reaching their personal growth goals. Communicates and coordinates all issues/information related to these activities. Reviews progress data and reports on outcomes towards goal areas. Supports culturally competent recovery-based practices and person-centered planning as a shared decision-making process.
