The Health Coach RN provides provide lifestyle health coaching with behavior modification to assist members in health self-management in order to enjoy a better quality of life.   This position relies on education, experience and judgment to plan and assist the member in accomplishing health goals.  Creativity and some latitudes are expected to complete the responsibilities of the role, performs a variety of tasks and works under general supervision.

Individual Risk Coaching

  • Assists employer health plan member health to set realistic holistic goals using system tools and judgement. Gain trust to assure disease based, risk or high utilization individuals set and meet or exceed targets.
  • Consistently measures and communicates with the individual on goal status and documents progress and measures of improvement using system tools. Process includes constant communication, psycho-social skills and disease management care plans such as obesity, diabetes and hyperlipidemia.
  • Coordinates care team around individual to assure obstacles to goals are removed including such activities as making appointments, assuring prescriptions are filled, directing care to physicians, specialists, EAP or other providers.
  • Participates in member Healthy Lives screenings in order to obtain biometric data. Documents biometric data appropriately in online tool. Health coach's individual on biometrics obtained at time of the screening.

Communication and Collaboration

  • Communicates effectively with the provider to assure that the panel of individuals is on track with health goals.
  • Assures care plans in the health coaching tool align with the provider treatment, including outcomes for measurement of disease, wellness and other goals.
  • Participates in Healthy Lives screenings to obtain member biometric data. Documents biometric data in online tool. Provides brief coaching session with individual based on biometrics obtained.
  • Assists with worksite assessments to include evaluating policies for vending, catering and dining as needed.
  • Produces and participates in the creation of wellness webinars; newsletter & presentation content; and other special projects as needed.
  • Make referrals and/or receive referrals with/to Care Management; In-Home Monitoring and RN Care Coordinator as needed.

Employer satisfaction

  • Document progress to goals in the health coaching tool for employer reports around compliance with wellness screening recommendations.
  • Estimate fact-based savings for individuals in order to report cost savings to employers due to weight loss and holistic approach to personalized health coaching.
