Position Overview

We are seeking a dedicated and qualified Licensed Clinical Social Worker (LCSW) to join our compassionate team at Sun Valley Consultation Services. This full-time remote position is an excellent opportunity for an LCSW who is passionate about delivering high-quality mental health care within a hospice setting. Work Environment

This is a remote position, allowing for flexibility in your schedule while continuing to provide essential care to our patients. You will be embedded within a hospice center, collaborating seamlessly with interdisciplinary teams that include physicians, nurses, and support staff. Key Responsibilities

Reporting to the Social Work Manager, your duties will include but are not limited to:

  • Conducting comprehensive bio-psychosocial evaluations to assess the needs of patients and their families, identifying barriers to treatment, and creating actionable care plans.
  • Implementing evidence-based therapeutic interventions tailored to individual patients, utilizing both individual and group therapy modalities.
  • Consulting with medical and support staff to ensure the development of a coordinated care program that fulfills the psychosocial needs of patients, enhancing their overall care experience.
  • Documenting treatment progress and social work activities in patients' medical records to comply with departmental standards and regulations.
  • Providing education to patients and families on managing the psychosocial aspects of illness and health, fostering understanding and adherence to treatment plans.
  • Coordinating community resources and advocacy services to support patient and family needs.
  • Maintaining compliance with all relevant laws and ethical standards pertaining to social work practice.
Patient Population / Client Focus

Your clinical practice will primarily serve patients within a hospice care setting, focusing on individuals facing serious illness and their families. This role not only requires clinical expertise but also a deep understanding and empathy for the unique challenges this patient population encounters, including end-of-life issues, grief, and loss.

Qualifications

The ideal candidate will possess the following:

  • A Master’s Degree in Social Work from an accredited institution.
  • A current and valid LCSW license from the State Board of Social Work Licensure.
  • Demonstrated knowledge of psychotherapeutic techniques and theories pertinent to the needs of the patient population.
  • A solid understanding of aging processes, human development stages, and cultural sensitivity to provide tailored care.
Preferred Experience

While not mandatory, candidates with the following experience will be given preference:

  • Six months to one year of clinical experience in a health or mental health setting, particularly with hospice or palliative care populations.
  • Experience in utilizing specific therapeutic methodologies relevant to mental health treatment for serious illness.

Compensation and Benefits
