Pediatric Transitional Nurse - Registered Nurse (RN) / Licensed Vocational Nurse (LVN)
North Houston, TX | Bilingual Preferred (Not Required) | Full Time
Pay Range:
  • LVN: $30-$35/hour
  • RN: $35-$42/hour

Angels of Care Pediatric Home Health is hiring Registered Nurses (RN) and Licensed Vocational Nurses (LPN) for our Pediatric Transitional Nurse Team to support medically fragile children and their families during critical transitions to home care. This role is ideal for nurses who are confident working with high-acuity patients and skilled in trach, vent, G-tube, and emergency management. Nurses in this position play a critical role supporting families, training caregivers, and ensuring safe continuity of care in the home environment.

What You'll Do
  • Provide skilled pediatric nursing 1:1 care in patients' homes
  • Perform patient assessments and ongoing evaluations -Administer medications, treatments, and therapies
  • Support hospital-to-home transitions for high-acuity patients
  • Educate and support parents, families, and caregivers
  • Assist with discharge support and care coordination
  • Train and support permanent field nurses during transitions
  • Provide first-shift and interim coverage as needed
  • Collaborate with Clinical Care Managers and interdisciplinary teams
  • Respond appropriately in emergency situations and changes in patient condition

Qualifications
Required:
  • Active Texas Registered Nurse or Licensed Vocational Nurse (LVN)/Licensed Practical Nurse (LPN) license
  • Current CPR certification
  • Ability to lift and assist pediatric patients as needed
  • At least 3 years of experience with Trach care, Ventilator management, and TPN, post-graduation
  • Emergency response management
  • Pediatric home health experience
  • High-acuity pediatric patients

    Schedule & Travel
    • 12 hour shifts (Days or Nights)
    • Flexible scheduling based on patient needs and Nurse availability
    • 5 working days per week, minimum 1 weekend day
    • On call and Staffing coverage responsibilities as needed
    • Local travel within approximately 30-40 miles
