Job description

Primary Care Case Manager - Part time position available with Health Access Project (HAP). Duties include providing primary care case management services for project-eligible clients, expanding HAP's network of volunteer family practice physicians, processing charity care/pharmaceutical applications, assisting HAP clients with social services available in the community, and provide quarterly data on clients served and their specific outcomes.


This position requires one of the following: (1) A Bachelor's degree in social work, psychology, public health, or a related field and at least one year of experience providing direct case management services to a comparable client base. (2) A minimum of 90 semester units with an educational major in social work, psychology, public health, or a related field and at least three ears of direct case management experience.

Bilingual skills in English and Spanish along with advanced computer literacy in Microsoft Office (Word, Excel, etc.) are required. Must be well organized, able to multitask, and have excellent written and verbal skills. Experience working in a health care setting with diverse populations is preferred.
