In this community nursing role, as a Registered Nurse (RN) you will have scheduled visits to care for patients in various settings. What you'll do:
  • Using appropriate assessment tools/techniques to assess patients' individualized needs
  • Executing and continuously monitor care plans
  • Engaging patients using a collaborative approach by building rapport and trust
  • Maintaining and update professional patient records related to patient encounters and plans
  • Assess patients to identify appropriate nursing interventions
  • Collaborate with members of an interdisciplinary health team to plan, implement, coordinate and evaluate patient care in consultation with patients and their families
  • Administer medications and treatments as prescribed by a physician or according to established policies and protocols
  • Monitor, assess, address, document and report symptoms and changes in patients' conditions
  • Operate or monitor medical apparatus or equipment
  • Assist in surgery and other medical procedures
  • May supervise licensed practical nurses and other nursing staff
  • May develop and implement discharge planning process on admission of patients
  • May teach and counsel patients and their families on health-related issues in collaboration with other health care providers
Job Requirements:
  • Nursing degree or required education mandatory for CNO licensure
  • Registration with the CNO as a Registered Nurse in good standing
  • Clear Vulnerable Sector Screen
  • Negative TB Test
  • Fluent proficiency in written and verbal English
  • Valid driver's license (Class G or G2), access to a dedicated vehicle is requiredand valid car insurance
  • Current and valid CPR certificate
  • Experience in delivering community care is an asset
  • Reliability and flexibility
  • Willingness to travel to and from patient residences within a designated area
  • Passion for making a difference in your community
  • Required on-call coverage, including after hours to ensure patient safety and to meet patient care needs
