Position: RN Psych, Home Health

Position Type: Full-Time

Responsibilities

An RN Psych Nurse is responsible for providing nursing care requiring specialized psychiatric and mental health clinical skills within the scope of Home Health to patients with physical and mental illness, late life depression and substance abuse concerns. Provides direct patient care under physician's orders utilizing nursing theory, skills and techniques. Provides instruction and supervision of LVNs/LPNs and Home Health Aides. Assumes responsibility for patient admissions, establishing plans of care, care coordination, recertifications and planning discharges.

If you meet these qualifications, we would love to meet you:

  • Current State RN license must meet one of the following qualifications:
    • Diploma or AA degree in nursing with at least 2 years recent nursing experience, recommended to be within the last 3 years, in a psychiatric or mental health home care program or an acute psychiatric or mental health hospital or outpatient program, or
    • Baccalaureate in nursing with at least one-year recent nursing experience, recommended to be within the last 3 years, in a psychiatric or mental health home care program or an acute psychiatric or mental health hospital or outpatient program, or
    • Master’s degree in psychiatric or mental health nursing with at least one-year recent nursing experience, recommended to be within the last 3 years, in a psychiatric or mental health home care program or an acute psychiatric or mental health hospital or outpatient program.
  • For agencies providing psychiatric services under the Medicare Home Health Benefit for which Palmetto is the Fiscal Intermediary, the resume of the Behavioral Health/Psychiatric Home Care Nurse that must be submitted to and approved by Palmetto.
  • Current CPR certification
  • Licensed driver with current auto insurance in accordance with state and/or organizational requirements and a vehicle in good working order.

Responsibilities:

As a RN Psych Nurse, you will:

  • Assessment of nursing needs, providing direct care and case management of the patient's care plan.
  • Makes initial assessment and reassessment visits utilizing observation/assessment skills.
  • Informs the physician of the results of the assessment and recommended clinical interventions.
  • Informs the physician and other personnel of changes in the patient's condition and needs.
  • Documents and submits the assessment, progress and discharge notes and other required paperwork in a timely
  • fashion as required by the agency.
  • Manages medical illness; performs psycho‐biological interventions.
  • Evaluates medicines to identify possible ineffective drug therapy, adverse reactions, significant effects, drug
  • allergies, side effects and contraindications.
  • Instructs regarding ordered medication regimen and reviews medications and compliance.
  • Administers IM medications.
  • Manages situational or other crises
