Registered Nurse - Home Care (Agency Staffing Support)Registered Nurses deliver skilled, compassionate care to clients managing the effects of aging, injury, surgery, or illness within their own homes and communities. The role will include:

  • Collaborating with the patient, family/caregiver, and interdisciplinary healthcare team to formulate an individualized plan of care based on thorough patient assessment
  • Evaluating the effectiveness of treatment plans and adjusting care in consultation with the patient, family/caregiver, and healthcare team
  • Acting as a strong patient advocate by protecting and promoting patient rights, dignity, and autonomy, especially in situations where the patient may be vulnerable or unable to advocate for themselves
  • Applying critical thinking and clinical judgment to recognize and respond to changes in patient status, drawing on critical care experience where applicable
  • Navigating complex family dynamics with professionalism and empathy, supporting families through difficult caregiving decisions while keeping the patient's best interests at the centre of care
  • Managing responsive behaviours associated with dementia and cognitive decline, using de-escalation techniques to maintain a calm, safe environment for patients, families, and self
  • Traveling within your region between patients' homes
  • Providing care across a variety of settings, which may include private homes, clinics, retirement homes, or schools
  • Remaining comfortable and professional when entering homes that may be cluttered, under-resourced, or otherwise not in ideal condition, and adapting care delivery to the environment while maintaining safety and infection control standards
  • Performing other clinical duties as required
  • Delegating tasks as appropriate and within your scope of practice

Assets:

  • Experience with wound care, IV medications and re-sites, catheter care, compression therapy/VAC, and ostomy care
  • Critical care background and/or experience managing complex, high-acuity patients
  • Experience working with dementia care and de-escalation strategies

Qualifications

  • Have one of the following qualifications:
    • Bachelor of Science in Nursing degree (BScN)
    • Bachelor of Nursing degree (BN)
    • Registered Nurse or Registered Practical Nurse in good standing with College of Nurses of Ontario
  • Experience as a visiting nurse, visiting registered nurse, visiting registered practical nurse, and/or community nurse considered an asset
  • Physical ability to stand, bend, walk, crouch, kneel and reach, ability to lift 50lbs (23 kg) independently, ability to assist clients with standing, walking, and sitting; ability to reposition clients in bed
  • Valid Standard First Aid and Level C CPR
  • Updated immunization records (including completion of a two-step tuberculosis test within the last 6 months)
  • Clear criminal background check, and Vulnerable sector check are required upon hire
  • Ability to interact with others in a caring, tactful and courteous manner
  • Excellent problem solving, time management, and crisis resolution skills
  • Excellent interpersonal and communication skills, both verbal and written
  • Valid G2/G driver's license and access to a reliable car
  • Ability to use phone applications

Requirements

  • Vulnerable Sector Check completed within the last six months
  • Submission of proof of COVID vaccinations
  • TB test completed within 6 months to a year
  • Immunization record
  • Up-to-date CPR and First Aid certification
