Job Summary

Provides timely and comprehensive behavioral health evaluations in the Emergency Department. Provides mobile assessment services for inpatient consult liaison team and contract facilities. Collaborates with providers and multidisciplinary team members. Communicates with physicians and treatment team to facilitate transitions of care, including hospital admissions both inside and outside Prisma Health. Manages and collaborates in discharge planning for patients and coordinates with agencies providing post hospital care. Maintains records and document all assessments. Manages statistical data.

Essential Functions

  • All team members are expected to be knowledgeable and compliant with Prisma Health's purpose: Inspire health. Serve with compassion. Be the difference.

  • Assesses patient's behavioral health/substance abuse needs in collaboration with EM providers, psychiatrists, and EM interdisciplinary team by obtaining collateral information from a myriad of sources. Identifies and refers patients/family to community resources when appropriate. Refers and coordinates transfers to IP psychiatric hospitals/substance abuse treatment centers when indicated. Provides ongoing case management and support of this patient population and their family while pending discharge disposition.

  • Assists patients to evaluate both their strengths and symptoms, and facilitate patients setting their own goals, and plan for appropriate services.

  • Prepares case history of patient's personal, social, and emotional development.

  • Provides face-to-face behavioral health evaluations in the ED setting.

  • Communicates patient and family issues to treatment team and acts as the liaison between other active services and/or services involved in patient's discharge plan.

  • Applies facets of Mental Health First Aid during times of crisis to patients.

  • Provides telephone assessment for professionals in the community to determine clinical appropriateness for services.

  • Assists patients in locating and utilizing community resources including legal, financial assistance, and other referral services.

  • Manages oversight and compliance of emergency psychiatric/chemical dependency hospitalization papers in accordance with state law. Assists Medical Providers in the completion of Inability to Consent documentation by identifying the appropriate surrogate decision maker in compliance with the Adult Healthcare Consent Act. v

  • Performs other duties as assigned.

Supervisory/Management Responsibilities

  • This is a non-management job that report to a supervisor, manager, director or executive.

Minimum Requirements

  • Education - Master's degree in Psychology, Rehabilitation Counseling or related field of study.

  • Experience - One (1) year of counseling experience.

  • LPC (Licensed Practical Counselor), LMFT (Licensed Marriage & Family Therapist), LAC (Licensed Associate Counselor), LPES (Licensed Psycho-Educational Specialist) or related licensure required.

Knowledge, Skills and Abilities

  • Skills necessary to assess patients of all ages.

  • Ability to communicate with appropriate nomenclature when reviewing patients with physicians.

  • Ability to complete Clinical Documentation on the computerized documentation system.

  • Emergency room experience preferred.

  • Group therapy, private practice exposure preferred.

  • Individual, family and marriage counseling experience preferred.
