Job description

Performs a wide variety of patient care tasks utilizing a Patient Focused Care model.  Follows standardized procedures prescribed in a clinical pathway, including activities related to direct care, diagnostic testing, patient transport and documentation, as directed by RN.

Essential Job Functions and Responsibilities:

  • Provides direct care to patients under supervision/direction of the registered nurse
  • Performs a wide variety of tasks using a patient centered model inclusive but not limited to:
    • Assist with Bathing / Personal Hygiene
    • Assists with ambulation of patient
    • Assists with feeding
    • Assist with toileting
    • Assists with vital signs
    • Weights
    • Safety checks
    • Continuous observation of patients
  • Exhibits knowledge and skills necessary to provide patient care
  • Exhibits knowledge of principles of growth and development for patient groups served
  • Demonstrates knowledge of providing care for specific population / age
  • Demonstrates knowledge of computer skills
  • Exhibits ability to provide safe care to individual needs of the patient / family / caregiver in a patient centered environment
  • Exhibits attributes of caring, empathy and promotion of positive work environment
  • Embodies a strong work ethic

Educational/Minimum requirements:

Must have High School Diploma or equivalent (GED).

Must have a valid and active CNA Certification in the State of Connecticut.

Must have a valid CPR w/BLS certification from the American Heart Association.

Must have a minimum of 1 year of experience working in a long term care and/or behavioral health setting.

Preferred:

  • Good communication skills
  • 1 year experience in acute care inpatient environment as Patient Care Assistant or CNA

State/Federal Mandated Licensure or Certification Requirements:

None

Bristol Hospital Mandated Educational Requirements:

General orientation at time of hire.  Accucheck, CPR, Fire/Safety/Infection Control annually. Health Care Provider CPR from American Heart Association or Professional Rescuer CPR by American Red Cross or other similar programs deemed appropriate by management.  Additional cross-training as appropriate to perform diverse job duties.  Successfully complete all training programs as determined by the Hospital.  Other unit based competencies/skills as required.

Special Requirements:

Knowledge of basic patient care procedures and medical terminology.  Ability to work as part of a team.  Data entry and documentation skills.  Experience working with a psychiatric population.

Physical Requirements:

Significant walking, bending, stooping, patient lifting and positioning.  Must be able to participate in PERT.  Manual dexterity for phlebotomy and data entry.

Work Environment:

Stressful working conditions in a patient care environment.  Exposure to patient fluids and waste.  Rapid pace and variable environment.

Cognitive Requirements:

Mental flexibility to perform diverse duties involved in patient care.  Good communication skills, written and oral, and ability to follow written and oral instruction.
