Job Description:
Summary:
The RN, CCC Coordinator serves as a key clinical professional responsible for facilitating patient flow and transfer process across the Ballad Health system. Under the supervision of the CCC Managers and/or CCC Supervisor, this role supports efficient patient throughput by coordinating bed placement, managing transfer logistics, and communicating with clinical teams across departments and facilities.
This position requires strong clinical knowledge, situational awareness, and the ability to multitask in a fast-paced environment. The coordinator ensures timely and appropriate patient placements while maintaining compliance with EMTALA regulations and organizational protocols.
Key Responsibilities
Patient Throughput & Transfer Coordination
• Coordinate inbound, outbound, and intra-system transfers, including direct admissions.
• Prioritize placement of emergent, urgent, and routine patients from hospitals, ERs, urgent care centers, long-term care facilities, and physician offices.
• Use bed management dashboards and transfer systems to support real-time capacity planning.
• Monitor patient flow metrics and promptly escalate delays or concerns to the appropriate clinical supervisor or manager.
• Collaborate with physicians, nursing leadership, and other stakeholders to facilitate safe and timely transfers and placements.
Clinical Communication & Coordination
• Act as a clinical resource and liaison between referring and receiving departments, physicians, and facilities.
• Accurately communicate patient condition, acuity, and needs to ensure appropriate placement.
• Triage and relay clinical presentations to physicians for consults and transfer acceptances.
• Maintain data accuracy in electronic health records and transfer/bed management systems.
Education, Compliance & Process Improvement
• Stay current on workflow updates, protocols, and software systems related to patient transfer and bed management.
• Adhere to EMTALA regulations, Ballad Health Code of Ethics, and compliance policies.
• Participate in ongoing training and quality improvement initiatives.
• BLS and ACLS if desired
Requirements:
• Bachelor's degree preferred, or in lieu of bachelor's degree, associates with a minimum of 5 years of clinical experience.
• Preferred background in acute care, critical care, patient placement, or transfer center operations.
• Strong clinical judgment, problem-solving, and communication skills.
• Ability to work collaboratively in a high-pressure, team-oriented environment.
• Familiarity with clinical software and real-time bed management systems is preferred.
