Baywood Court is actively recruiting for a Social Services Designee in our Health Center (Skilled Nursing Facility). The Social Services Designee plays a vital role in supporting residents and their families by addressing psychosocial needs, easing transitions into the care environment, and ensuring thoughtful, wellcoordinated discharge planning. This position serves as a trusted advocate and resource, helping residents maintain dignity, wellbeing, and continuity of care throughout their stay.

Working collaboratively with residents, families, and the interdisciplinary care team, the Social Services Designee contributes to personcentered care planning and problemsolving while navigating complex emotional, social, and regulatory considerations. This role is ideal for a compassionate professional who values meaningful relationships, teamwork, and resident advocacy, and who is committed to excellence and compliance with state and federal regulations, including Title 22 and CMS requirements.

FLSA Status: Non-Exempt, full-time. Hourly pay range: $30 - $35; Mostly Monday to Friday

We are a 24/7 facility therefore one must be able to work weekends and/or holidays as required. All applicants are required to complete and pass a drug screening, physical exam with tuberculosis (TB) skin test, and criminal clearance through FBI and Department of Justice. The background search includes minor misdemeanors and DUIs.

ESSENTIAL DUTIES:

  • Conduct psychosocial assessments for residents upon admission, quarterly, annually, and as needed.
  • Participate in interdisciplinary care plan meetings and contribute to individualized care plans. Identify psychosocial, emotional, and discharge planning needs of residents.
  • Provide counseling, emotional support, and crisis intervention to residents and families.
  • Facilitate communication between residents, families, and facility staff.
  • Address resident concerns and grievances in accordance with facility policies.
  • Coordinate safe and timely discharge planning, including referrals to community resources.
  • Facilitate transitions of care to hospitals, home health, assisted living, or other settings.
  • Connect residents and families with community resources such as financial assistance, transportation, and mental health services.
  • Advocate for residents’ rights and ensure respect, dignity, and self-determination.
  • Ensure compliance with federal (CMS), California Department of Public Health (CDPH), and Title 22 regulations governing social services.
  • Bachelor’s degree in Social Work or a related human services field preferred OR
  • Licensed Social Worker (LCSW) or Associate Clinical Social Worker (ACSW) (not required but preferred) OR
  • High school diploma (or equivalent) and at least 5 years of supervised social services experience in a healthcare setting. Must possess Social Services Designee Certificate
  • Proficiency with computers and working knowledge of Microsoft Office applications and MatrixCare preferred.
  • Knowledge of SNF regulations (CMS, OBRA, Title 22)
  • Experience in a skilled nursing facility or long-term care setting
  • Bilingual (e.g., Spanish, Mandarin, etc.) depending on resident population (not required but preferred)
  • Strong assessment and care coordination skills
  • Ability to manage multiple priorities in a fast-paced environment
  • Compassion, empathy, and strong advocacy mindset
  • Knowledge of community resources and discharge planning processes
  • Conflict resolution and problem-solving skills
  • Ability to read, write and speak English clearly using proper speech and grammar.
  • Ability to communicate clearly, accurately, tactfully, and effectively with employees, residents, families and physicians while maintaining a professional demeanor.

PHYSICAL DEMANDS/WORKING CONDITIONS:

Must be able to move freely throughout the facility, and lift up to 50lbs with assistance. Prolonged, extensive, or considerable standing/walking and sitting. Frequent contact with residents under wide variety of circumstances. May be exposed to unpleasant  conditions including illness or injury.  (accidents, injuries, illnesses). OSHA Risk Category II – Medium Risk.
