Reports to: Director, CTTP
Location: Basile Rowe, East Syracuse, NY 13057
Pay Rate:$70,720 - $74,880
Schedule: Monday - Friday: 8 a.m. - 4 p.m.

Our focus is supporting individuals by ways of residential housing, mental health and clinical services, and immersing our participants in community engagement. Working closely with the Office for People with Developmental Disabilities and the Office of Mental Health, we support 27 housing locations and 3 administrative buildings.
About the Role:
We are looking for a proactive employee to provide clinical and therapeutic services to children with a range of mental health concerns and/or intellectual and developmental disabilities, in the Critical Time Transition Program
Responsibilities:
o Complete initial evaluations and re-evaluations for assigned individuals on CTI caseloads
o Use professional judgement and training to develop goals for assigned individuals
o Use best practices in performing/implementing counseling services
o Maintain appropriate contact with assigned individuals, their families, guardians, and care managers
o Perform home evaluations, sessions, or training, as needed
o Maintain full caseload; schedule make-up visits for cancellations in order to provide adequate support during time of transition for participant
o Utilize proactive approaches to avert and/or de-escalate crisis situations
o Participate in the staff on-call rotation, providing after-hours support and interventions via phone, as needed
o Provide information and resources regarding diagnoses, disabilities, and treatment
o Conduct self in a professional manner
o Attend staff meetings and other meetings as directed by supervisor
o Participate in in-service programs
o Participate in Agency-wide events, advocacy, and/or, community education
o Provide support to other Agency programs, as needed
o Provide person-centered, quality, services
o Demonstrate effective time management skills
o Observe confidentiality and safeguards regarding all participant related information
o Follow Agency standards and procedures
o Maintain compliance with applicable program rules and regulations
o Contribute to department and Agency quality improvement initiatives
o Ensure compliance with federal, state, and local employment laws and regulations, as well as with company policies
o Perform Other Duties as Assigned
Education/Experience:
Master's degree in social work or Mental Health Counseling, required. Provisionally licensed in NYS (LMSW, LCSW, or LMHC) and working to obtain licensure, required. Currently licensed, preferred. One year of experience working with children and adults with developmental disabilities and/or mental health diagnoses, preferred.
