As a Home Health Registered Nurse (RN) Caseย Manager,ย hereโ€™s a big picture view of what youโ€™ll do:

  • Oversee the delivery of clinical and caregiver services, ensuring the highest quality of patient/client care in accordance with home health laws and regulatory standards
  • OASIS admissionsย and Coordinate with a team of LPNs, ย CNAs, aides and to ensure client and patient goals are met
  • Follow the Home Health orders from PCPs working closely with their office
  • Ensure medical supplies/equipment are delivered and staff are trained on care procedures
  • Assess patient/client needs, resolve issues, and ensure a high level of satisfaction
  • Effectively manage financial resources related to the delivery of quality patient/client care

A few must-haves for a Home Health Registered Nurse (RN) Case Manager:

  • Graduate of an accredited nursing program and activeย RN or BSNย in Massachusettsย 
  • Minimum of 1 year of nursing experience in home healthcare/ย OASIS admissions, recerts, resumptions and DCโ€™s
  • At least 1 year of supervisory and caregiver management experience
  • CPR certificationย 
  • Extensive knowledge of state and federal home health laws and regulatory standards
  • Excellent management, organizational, problem-solving, communication and interpersonal skills,ย activities that may be required.
