Summary:
The Ballad Health Pharmacy Services Clinical Coordinator shall be responsible for oversight of the provision of clinical pharmacy services/pharmaceutical care as delineated by the service's Scope of Care for the facility patients of Ballad Health. With a primary focus on standardization and adoption of pharmacy best practices across Ballad Health, activities include the following:
• Provide oversight of the Ballad Health medication formulary and ensures cost-effective utilization.
• Designs and implements evidence-based medication use processes that provide optimal patient outcomes that are consistent with currently accepted standards of practice.
• Responsible for coordination of Pharmacy & Therapeutics Committee activities, including serving as Secretary for that Committee.
• Works to promote the use of established medication use policies and assists with the identification and development of new policies within the Department of Pharmacy and/or the Pharmacy and Therapeutics Committee.
• Collaborates with directors and managers of Ballad Health facilities to plan performance improvement activities.
• Position requires the ability to communicate and collaborate effectively with patients, physicians, nurses, pharmacists and allied health professionals from all clinical disciplines regarding activities related to the improvement of medication use.
• Mentors and serves as a clinical resource for pharmacy staff and students.
• Ensures that all activities are consistent with the overall organizational objectives.
Responsibilities:
• Provides oversight in planning, development, maintenance, and ongoing review of a formulary that promotes rational, cost-effective drug therapy across the organization. Ensures that formulary changes are communicated to appropriate providers and staff in a timely manner.
o Prepares drug monographs and reviews drugs for formulary consideration.
o Oversees and/or contributes to newsletter articles that provide pertinent medication-use and safety information for healthcare professionals.
o Assists in planning, development, and implementation of Medication Use Evaluations (MUE).
• Participates in the development and implementation of clinical pharmacy goals that are in alignment with the organization's mission, values and strategic plan.
• Collaborates with pharmacy management team and provides over-sight in the planning, development and implementation of departmental quality improvement efforts to ensure positive outcomes.
• Provides advice and guidance to medical staff and other clinical providers on the provision of evidence-based medication therapies and protocols with regards to age-specific populations.
• Communicates with medical, nursing, pharmacy and ancillary staff in a positive and effective manner. Communicates and collaborates with hospital leaders and staff to ensure timely and accurate exchange of information across the organization.
• Plans, develops and implements or modifies clinical services to meet new or unmet needs. Collaborates with management team to resolve all identified problems and adopt preventative actions.
• Actively and effectively participates or provides oversight of participation in organizational standing and ad-hoc groups concerned with medication utilization and safety to define present and future needs and to assist in problem resolution.
• Develops positive relationships in satisfying the needs of internal and external stakeholders.
• Supports the mission of the Department of Pharmacy Services in the training and guidance of pharmacy students and residents. Serves as preceptor in the area of specialty. Attends and participates in education-related committee meetings. Ensures that evaluations are appropriate and that all accrediting standards are met.
• Assists in the planning of departmental goal and new services . He/she will ensure systems and processes for effective communication and engagement of the workforce to communicate key decisions and encouraging frank two way communication regarding the medication use process. He/She should manifest good communication and interactive skills, a positive outcome/results orientation and the ability to function successfully as part of multidisciplinary teams, as well as independently to develop and foster collaboration to ensure the integrated delivery of services. He/she shall demonstrate leadership qualities, routinely serve as a role model, and shall provide guidance and serve in a consultative role.
Pharmacy Services in Tennessee & Virginia Facilities will be managed via a shared governance structure in which the Ballad Health Clinical Coordinator shares/carries out expanded/management functions. These functions will be delineated by the Ballad Health Director of Pharmacy Services.
Requirements:
Pharmacy Services Clinical Coordinator must be a graduate of a college of pharmacy accredited by the American Council for Pharmaceutical Education. Education and training should include capabilities/competencies to provide pharmaceutical care for patients of all age groups.
The Ballad Health Clinical Coordinator will have five (5) years of clinical pharmacy experience in an acute care setting. His/her clinical experience shall include, but not be limited to, clinical consultation/review, medication use evaluation for patients of all age groups, nutritional support, and pharmacokinetic dosing. He/She must have past supervisor/ management experience. Capabilities/knowledge must also be exhibited in the use of related information systems hardware/software. Previous experience/proficiency in the provision of education and training for pharmacy students and residents plus healthcare providers is needed.
An advanced pharmacy degree with residency and Board Certification in Pharmacotherapy (BCPS) shall be required. This individual shall possess good communication skills and analytical and leadership abilities. He/She must demonstrate the ability to work closely with physicians and paramedical professions in a multidisciplinary setting.
