Job description

What You’ll Be Doing

You will complete the Wheels of Change and field training to become a certified FCT and FCT-R Practitioner. The FCT-R Practitioner will provide in-home mental health and/or substance use treatment with the intent to keep families together, based on the Family Centered Treatment Model. You will support clients and families on their mental health and/or recovery journey by collaborating with a team of practitioners and Peer Recovery Specialists to promote recovery, stability, and the ability to maintain the family within the home. 

Hybrid Role, 20% Remote Work. 

Your Main Priorities

While additional responsibilities are likely to arise on the job, you will primarily:

•    Work with clients and their family members and caregivers to identify opportunities and goals, assess functioning, develop a treatment plan, and assist in achieving desired outcomes. 

•    Build and maintain positive relationships with clients, family members and caregivers, and identify referring or collateral professionals and agencies. 

•    Collaborate with collateral contacts as needed. 

•    Provide two 2-hour treatment sessions per week with each client/family assigned.

•    Flexible in providing services according to the client’s needs. 

•    Maintains client records accurately, completes documentation requirements timely. 

Who You Are

Requirements, Skills & Abilities:

•       Bachelor’s degree in social work of related field required; Master of Social Work degree preferred. 

•       Minimum 1 year experience in a similar role required; experience with trauma-informed and substance use treatment a plus. 

•    On call as needed. 

•    State wide travel is required. 

•    Strong organizational, critical thinking, and time-management skills.

•    Basic computer skills required; knowledge of Microsoft Office 365.

•    Ability to communicate and effectively interact with a diverse population.

•    Bilingual in Spanish preferred. 

•    Collaboration and teamwork. 

•    Must authorize Child & Family to complete a motor vehicle records check.

•    Must have valid driver's license, auto insurance and car registration.  

•    The ideal candidate will be required to meet all background checks and medical clearances required for the position. 

Physical Requirements

Direct Service Staff:

This position requires time sitting, standing, walking, driving, carrying and lifting up to 25lbs. This role also requires operating desktop, laptop and/or cell phone, and communicating with colleagues.
