Job description

Hospital Social Worker

The Social Worker provides ongoing support and expertise through comprehensive assessment, planning, implementation and overall evaluation of individual patients and family needs. The Social Worker will participate in the provision of bereavement services as needed.
Schedule: Part-time (24 hours per week), working 3 days per week from 8:30 a.m. - 5:00 p.m. Includes every 4th weekend rotation and one holiday every other year.
What you'll do
As a Social Worker, you will play an essential role on our interdisciplinary team, supporting patients and families through the end-of-life journey. Your work includes:
  • Completing initial psychosocial assessments and establishing individualized plans of care.
  • Providing emotional support, education, crisis counseling, and advocacy.
  • Communicating regularly with patients, families, caregivers, and facility partners.
  • Assessing bereavement risk and supporting grief services as needed.
  • Coordinating community resources and referrals to ensure safe, appropriate care in the home.
  • Assisting families with funeral planning upon request.
  • Participating in IDT meetings, plan-of-care reviews, and team collaboration.
  • Working with Volunteer Services to align volunteers and coordinate Veteran pinning ceremonies.
  • Supporting process improvements and using Unity's specialty programs (Gifted Wishes, Holistic Harmonies, Respecting Valor).
  • Maintaining accurate, timely documentation and adhering to the Social Work Code of Ethics.
  • Participating in the 24/7 coverage rotation and maintaining dependable attendance.

Requirements
Knowledge, Skills, and Abilities:

  • Satisfy mandatory training requirements regarding local, state, and federal regulations and/or Unity's policies and procedures.

Minimum Qualifications & Certification/Licensure:
1. Bachelor's in Social Work (BSW) from an institution accredited by the Council on Social Work Education; or bachelor's degree in psychology, sociology, or other fields related to social work, along with one year in a healthcare setting. Master's of Social Work preferred.
2. Certification or licensed as a social worker (CSW, CAPSW, CISW, or LCSW) by the State of Wisconsin is required.
3. Valid Wisconsin Driver's License, reliable transportation, and proof of current automobile insurance coverage.
