Location: Glendale, AZ


Specialty: RN Case Manager NE
Assignment Length: 13 weeks with possible extension
Hours per Week: 36
Schedule: Days
Shift Time: 9:00 AM – 5:30 PM
Shift Details: Shift time:9am- 5:30 pm  Shift days: M-F and every third weekend
Travel Pay Estimate: $2,187.00/week
Additional Information:
Licensed master’s level social worker in state of AZ OR RN State of Arizona,  At least five years of Case Management experience in an acute care hospital,  Assist with Initial Assessments, agreeability and DC plan with post-acute preferences.  Heavy in DC planning experience
