Nurse Practitioner Opportunity at Conviva Primary Care

You will work on-site at our Winter Haven, FL clinic. You will report to the associate medical director. Join a team that's redefining senior primary care. We are looking for a nurse practitioner with an autonomous license to join our team full-time at Conviva Primary Care. This role is ideal for a compassionate, experienced clinician who excels in a collaborative, patient-centered environment and is committed to improving outcomes for adult and geriatric populations.

Main Responsibilities

  • You will deliver comprehensive outpatient care to adult and senior patients.
  • You will maintain accurate medical records and documentation.
  • You will diagnose and manage moderately to complex medical conditions.

  • You will participate in clinical quality improvement projects.
  • You will work with interdisciplinary teams to ensure holistic care.

Required Qualifications

  • Three years of experience in value-based care or managing adult patients with chronic medical conditions
  • Masters' degree as a nurse practitioner from an accredited medical school.
  • Active, autonomous nurse practitioner medical license in the state of practice, or to obtain
  • Board Certification
  • If selected for this role, you will be required to be screened for TB

Must be willing to participate in limited on-call coverage by phone

Scheduled Weekly Hours: 40

Pay Range: $136,200 - $187,400 per year

This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
