Job description

Easterseals is seeking an experienced and motivated School-Based Physical Therapist Coordinator to lead and support physical therapy services in a school base setting. This role is ideal for a licensed Physical Therapist who thrives in a leadership role and is passionate about helping children succeed in a school-based setting.
What You'll Do

  • Conduct initial and ongoing evaluations, maintaining accurate documentation and progress notes.
  • Develop individualized, goal-driven treatment plans and reassess students according to established timelines.
  • Collaborate with interdisciplinary teams to optimize scheduling and maximize therapeutic outcomes.
  • Oversee scheduling for evaluations, referrals, and screenings.
What You Bring
  • Active Illinois Physical Therapist license.
  • Minimum of 2 years of experience in a school-based physical therapy setting.
  • Current Child CPR and First Aid certification (or willingness to obtain).
  • Prior supervisory or leadership experience preferred.
  • Strong organizational, communication, and collaboration skills.
