Social Worker (Patient Care Manager - PCM)
Aurora Behavioral Health | Glendale, Arizona
Join a Team That Changes Lives
Aurora Behavioral Health is seeking a compassionate, motivated, and patient-centered Social Worker (Patient Care Manager) to join our multidisciplinary team in Glendale. In this role, you will provide clinical assessments, individual and group therapy, treatment planning, case management, and discharge coordination for patients receiving inpatient behavioral health services.

What You'll Do
As a Patient Care Manager (PCM), you will:
  • Complete comprehensive psychosocial assessments.
  • Develop, implement, and update individualized treatment plans.
  • Facilitate individual, family, and group therapy sessions.
  • Coordinate discharge planning and connect patients with community resources.
  • Collaborate with psychiatrists, nurses, behavioral health technicians, and other members of the interdisciplinary treatment team.
  • Document assessments, treatment plans, and patient progress in the electronic medical record.
  • Participate in utilization review and ensure timely documentation.
  • Advocate for patients while maintaining the highest standards of confidentiality and ethical practice.
  • Assist patients and families in achieving treatment goals and successful transitions of care.

Qualifications
Education
  • Master's degree in social work, Counseling, Psychology, or another behavioral health-related field.

Licensure
We welcome applicants at various stages of licensure:
  • LMSW
  • LCSW

  • LAMFT
  • LMFT
  • LASAC

Clinical supervision is available for associate-level clinicians.
Requirements
  • Current CPR certification (or obtained within 30 days of hire)
  • Current TB clearance
  • Arizona Fingerprint Clearance Card
  • Must be at least 21 years of age
