Description:

The MDS Care Plan Coordinator performs nursing assessments of residents and documentation review, and analysis as needed for completion of resident’s MDS, CAAs and Care Plan in accordance with standards of nursing practices, regulations, and RAI Manual. May lead or direct licensed and non-professional facility staff in the assessment and competition of MDS, CAA and Care Plan process.


Responsibilities:

• Maintain and update schedules of MDS’s that are due.

• Coordinate the work of the Interdisciplinary Team to complete the MDS, which includes the CAAs and care plan.

• Enter or supervise input of MDS into the computer.

• Act as the leader of the MDS and care plan meeting.

• Body Audit/ Diagnosis/ Orders

• Coordinate the completion of the care plan, except in those facilities who have a separate Care Plan Coordinator.

• Assure that all signatures are present on MDS and care plans.

• Monitor QIES website for CASPER reports and initiate correction action and reports as needed.

• Assist in audits, denials and ADRs as requested by CES.

• Assist in education of facility staff as it relates to the RAI process and compliance, as needed.

• Any other duties as assigned by the Administrator.

Requirements:

EDUCATION and/or EXPERIENCE

• Associates, Bachelor’s degree or university program certificate.

• 2-4 years related experience and/or training or equivalent combination to education and experience required.

• Experience as an MDS/Care Plan Coordinator preferred.

• LPN or RN License

• Long term care experience preferred.
