Job description

Home Health RN - Day shift with rotating weekends.
Essential Functions:
1. Performs initial and follow-up care to homebound patients, coordinates treatment programs based on evaluative findings and revises nursing care when patient status changes.  Administers nursing services as ordered by physician.    
2. Participates in departmental activities to promote continuous quality improvement and maintain productivity at budgeted levels.    
3. Directs and supervises the application of nursing services by supportive personnel, including Home Health Aides.    
4. Ensures the best possible care by communicating with client and/or family and meets client’s need for courtesy, information, physical care, emotional care, safety, privacy, and confidentiality of patients undergoing care by Home Health Services.    
5. Prepares, submits, and maintains patient records and reports as required by departmental , hospital, state, and national standards, including Home Health charges and progress notes.    
6. Communicates with physicians and other healthcare personnel regarding patient progress, treatment programs, or other pertinent information.    
7. Follows all infection control procedures and policies (including use of universal/standard precautions).    
8. Reports significant client status changes to Primary Care Physician.    
9. Performs activities related to necessary ongoing operation of the department: a) ordering supplies; b) restocking of supplies; c) cleaning; d) filing information.    
10. Markets the service to physicians & the community through meetings, seminars & health fairs.    
11.  Performs other duties as assigned.    

Non-Essential Functions:
12. Serves as a resource for professional knowledge concerning Nursing Care and Services.    
13. Arranges and maintains daily schedule of patients.    
14. Acts as a clinical instructor for Nursing and other programs sending students to facility.    

Qualifications:
Knowledge, Skills, and Abilities:        
Excellent communication skills, must have the ability to communicate well with clients and co-workers.  Possess skill in analytical thinking, planning, and organization.        
Education:        
Associate or Bachelors Degree in Nursing from accredited and   approved school of Nursing.        
Experience:        
One year experience in Home Health preferred.        
Certification/Licensure:        
Maintains current license as a Registered Nurse in the State of Kansas.          
Code Blue Training:        
Must be BLS certified.        

Physical Demands:
May require frequent sitting, standing, walking, bending, and grasping.  Occasionally requires lifting and carrying up to 55 lbs.  May also require pushing and pulling up to 55 lbs.  Job requires full range of body motion, with occasional twisting, climbing, balancing, stooping, kneeling, crawling, and reaching.  Job may require reaching overhead.  Job requires ability to handle and lift clients.  Eye-hand coordination, and finger and manual dexterity are required.  Requires corrected hearing and vision to a normal range.  Job requires working under stressful conditions and/or irregular hours.  Some exposure to communicable diseases, sharps, and hazardous material is anticipated.  Job requires potential exposure to body fluids and radiation.  Requires the ability to distinguish letters and symbols.  Use of office equipment such as telephones, computers, copying machines, and intercom may be required.  Repetitive movements such as writing or typing is required.

Working Conditions:        
Must be available to work weekends and holidays, with potential for varied shifts, overtime, cost containment absent days, or standby time.  Must comply with uniform requirements.
