Home health agency servicing patients in the Chicagoland area with home nursing and therapy needs.

Job Summary

Responsibilities:

- Patient assessment and observation, administration and monitoring of prescribed medication, treatments and therapies in patient's home environment

- Follow designed Plan of Care

- Communication with other members of the patients' multi-disciplinary team

- Prepare clinical notes and update primary Physician

- Health promotion and teaching

Qualifications

Requirements:

- Graduate of accredited school of nursing; LPN Program

- Current and unrestricted licensure in IL

- Valid driver's license and auto insurance

- Be able to work and travel independently

- Knowledge of basic computer operations such as typing, Windows OS, internet, and email

- CPR certificated

- Excellent observation, verbal, and written skills

- At least 3 year of clinical patient care experience in hospital or other clinical setting

- Preferred at least 1 year home health visiting experience
